 RELEASE
WAIVER
Online registrants will be expected to sign
release waiver
listed below on May 1st prior to
participation in the event.
ADULT RELEASE FORM
I hereby release St. David's Episcopal
Church of Kinnelon, NJ and their employees
and agents for all claims on account of any
injuries which may be sustained by me and/or
my minor son/daughter while participating in
the Brake the Hunger Cycle Tour bike event
on May 1, 2010 and any such claims which
hereafter may be presented by me and/or my
minor son/daughter as a result of any such
injuries. I also certify that I and/or my
minor son/daughter are medically fit to
participate in the Brake the Hunger Cycle
Tour bike event. I hereby authorize the
sponsors of the Brake the Hunger Cycle Tour
bike event to act for me or my minor
son/daughter according to their best
judgment in any emergency requiring medical
attention.
MINOR RELEASE FORM (IF APPLICABLE):
I, the minor’s parent and/or legal guardian,
understand the nature of bicycling
activities and the minor’s experience and
capabilities and believe the minor to be
qualified, in good health, and in proper
physical condition to participate in such
activity. I hereby release, discharge,
covenant not to sue, and agree to indemnify
and save and hold harmless each of the
releases from all liability, claims,
demands, losses, or damages on the minor’s
account caused or alleged to be caused in
whole or in part by the negligence of the
“releases” or otherwise, including negligent
rescue operation and further agree that if,
despite this release, I, the minor, or
anyone on the minor’s behalf makes a claim
against any of the releases named above, I
will indemnify, save and hold harmless each
of the releases from any litigation
expenses, attorney fees, loss, liability,
damage or cost which any may incur as the
result of any such claim.
